Solution retrospective

Refiz o formulário três vezes até chegar à versão final que me agradou. Embora ainda haja espaço para muitas melhorias, estou contente com o resultado alcançado no último formulário. Estou aberto a feedbacks.

I revised the form three times until I reached the final version that I liked. Although there is still room for improvement, I am happy with the result achieved in the last form. I am open to feedback.
