Special shoutout to @zoulizhu's solution on the same project. Their solution helped me construct the form.

It would be appreciated if someone can review my javascript. It is probably possible to refactor the code, but I am unsure how!

Otherwise, I know I need to work on forms. They continue to pose me challenges.
